The most challenging key point in thyroid surgery should be the protection of bilateral recurrent laryngeal nerves, the nerves underneath  bilateral thyroid glands that control the movement & phonation of vocal cords. We insist the fiberscopic examination of vocal cords at OPD before & after thyroid surgery , that take less than 1 minute to complete the vocal cord examination.
